Methodology

Points-deduction from 100. Each violation cited on the CAL CODE Official Inspection Report (Form 5191, ver 8/16) carries a fixed point deduction. Items 1-23 are CDC risk-factor and FDA public-health-intervention items: MAJOR violation deducts 4 points, OUT (minor) deducts 2 points. Items 24-54 are Good Retail Practices (GRPs): OUT deducts 1 point, with one local override -- Item 48 (Food Handler Cards) deducts 3 points per SBCC 33.0409(d). Remaining score determines letter grade: A 90-100, B 80-89, C 70-79 (re-score required within 30 days), Closure 0-69. Authority: California Retail Food Code (Cal Code) + San Bernardino County Code Chapter 33 (Inspection Grading of Food Facilities, Sections 33.0401 et seq. and 33.1401 et seq.).
